THE BEAUTY LOUNGE
Content Creation Model Interest Form
Thank you for your interest in being a model for The Beauty Lounge's upcoming skincare content creation day!
We are looking for models to help us create professional photos and videos featuring select skincare treatments. This is a content/modeling opportunity and is NOT a full facial service.
Treatment time will vary depending on the service and the content we need to capture. You may be asked to remain in one position, repeat certain steps, or allow us to photograph/video your skin before, during, and after the treatment.
Please answer all questions honestly and completely. Your safety comes first. Completing this form does not guarantee selection. The Beauty Lounge will review your information and determine whether you are an appropriate candidate for the treatment selected.
